Winter Camp 2025 Assumption of Risk, Consent & Liability

In consideration of my child's participation in the Winter Camp 2025 offered by Busy Minds Education®, I, the undersigned parent or legal guardian, hereby agree to the following terms and conditions:

Acknowledgment of Risks:

I acknowledge that participating in the Winter Camp 2025 involves physical activities, including but not limited to running, jumping, climbing, and balancing, as well as other structured recreational and educational activities. I understand that while every effort will be made to create a safe and supportive environment, there is always a possibility of injury or unforeseen circumstances.

Medical Clearance:

I certify that my child is in good physical and mental health and is capable of participating in the Winter Camp 2025. If my child has any pre-existing medical conditions or concerns, I have consulted with a healthcare professional and obtained their clearance for participation.

Supervision:

I understand that my child will be under the supervision of qualified Busy Minds Education® instructors and staff during Winter Camp 2025. I agree to inform the instructors of any special considerations or conditions that may affect my child's participation.

Code of Conduct & Dismissal Policy:

I acknowledge that all children attending Winter Camp 2025 are expected to follow the camp's behavior guidelines. Any child who engages in inappropriate behavior, including but not limited to the use of inappropriate language, physical aggression, bullying, or actions that cause harm to other campers or staff, may be immediately dismissed from the program without warning. 

Release of Liability:

In consideration of my child's participation in Winter Camp 2025, I hereby release, discharge, and hold harmless Busy Minds Education®, its instructors, employees, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action, arising out of or related to any injury, illness, or damage that my child may sustain while participating in the camp.

Emergency Medical Care:

I authorize Busy Minds Education® and its representatives to seek emergency medical care or treatment for my child in the event of an injury or medical emergency. I understand that Busy Minds Education® will make reasonable efforts to contact me or the emergency contact person listed, but I consent to medical treatment being administered if deemed necessary by medical professionals.

Photography:

I allow Busy Minds Education® to share photos or videos of my child participating in the program for promotional purposes. Please send an e-mail to ottawa@busyminds.ca to request otherwise.

By signing below, I confirm that I have read and understand this liability waiver and refund policy, and I voluntarily agree to its terms and conditions. I also affirm that I am the legal guardian or parent of the child participating in Winter Camp 2025.
