RESPONSIBILITY AND ACCEPTANCE OF RISKS

TO AIDA INTERNATIONAL AND TO THE AIDA INSTRUCTOR

I, the undersigned ____________________________________________________________________________ hereby certify on my honor that I am aware of the inherent risks associated with the practice of freediving, which can lead to serious injuries or death. Nevertheless, I choose to practice freediving with _________APNEACITY INC______________.
I understand and agree that neither my instructor ______APNEACITY INC____________ nor AIDA International, nor any of their employees, officers, contractors, or respective rights holders (hereinafter referred to as the “Released Parties”) can be held responsible or liable for any injury, death, or other damage that may occur to me, my family, my property, heirs or rights holders as a result of my participation in freediving activities with AIDA International, or as a result of negligence by any party, including the Released Parties, whether they have a passive or active role.
In consideration of AIDA International allowing me to participate in freediving activities, I hereby personally assume the risks of this experience, whether foreseen or unforeseen, that may occur while I am practicing freediving with ____________APNEACITY INC_______________.
I attest that I am in good physical and mental condition for practicing freediving and that I am not under the influence of alcohol or any substances or treatments contraindicated for practicing freediving. I further attest that if filling out the AIDA Medical Certificate was required, I have consulted a physician and obtained their approval to practice freediving.
I also attest that I am of legal age and legally responsible for signing this liability waiver. I understand that the terms herein constitute a contract and are not merely a preamble and that I have signed this document voluntarily and with full awareness that by doing so I waive my legal rights. I also agree that if any provision of this Agreement is found to be unenforceable or invalid, it shall be removed from this Agreement. The remainder of this Agreement shall then be interpreted as if the unenforceable clause had never been part thereof.
