Assumption of Risk & Release of Liability – Yalla FC
I understand that participation in Yalla FC involves physical activity and carries certain inherent risks, including but not limited to slips, falls, collisions, weather-related hazards, and other potential injuries (such as sprains, fractures, or concussions). I voluntarily assume full responsibility for any and all risks of personal injury, property damage, or loss that may occur as a result of my child’s participation.

In consideration of participation in Yalla FC, I hereby release and hold harmless Yalla Kids Asc., its owners, directors, employees, volunteers, coaches, and agents from any and all claims, liabilities, demands, or actions arising from or related to any injury, loss, or damage sustained during the program.

I confirm that the participant is physically able to take part in soccer activities and that I have disclosed any relevant medical conditions to Yalla Kids Asc. I also understand that Yalla Kids Asc. is not responsible for lost or stolen personal items.

In the event of a medical emergency, I authorize Yalla Kids Asc. staff to arrange for medical treatment and accept full responsibility for any related costs.

Code of Conduct Agreement

By participating in any Yalla program, members agree to:

Show respect toward all participants, coaches, referees, and staff at all times.

Follow all rules and instructions provided by Yalla staff for safety and enjoyment.

Use equipment properly and only for its intended purpose.

Maintain a positive, encouraging environment for all players.

Refrain from disruptive, aggressive, or unsafe behaviour.

Failure to follow the Code of Conduct may result in suspension or termination of membership without refund.
