Waiver/Consent

It is a condition to the Child's participation in swimming lessons that you, the undersigned Parent/Guardian of the Child, must carefully read and understand this document, and sign it to acknowledge that you have read and understand its contents and are fully aware that the Child's participation swimming lessons will expose the Child to risks of harm and that you accept full responsibility for exposing the Child to such risks.

In addition, by signing below, the Participant or the Participant’s Guardian understands, acknowledges, and assumes the inherent risks in participating in the Event, including, but not limited to: the potential for bodily injury or illness (including contraction of COVID-19); contact or interaction with others who may have been exposed to COVID-19; permanent disability, paralysis, or loss of life; collision with natural or manmade objects; dangers arising from adverse weather conditions; imperfect venue or field of play conditions; equipment failure; participants of varying skill levels; circumstances known, unknown or beyond the control of the Organizer, its sponsors, employees, or volunteers (together, the “Organization”); negligence or omission of the Organization (collectively, the “Risks”).

In consideration for allowing the Participant to participate in swimming lessons, the person signing below (the Participant or the Participant’s Guardian), I hereby release, discharge, indemnify the Corporation of the City of Elliot Lake, its employees and volunteers from any and all claims or any damages whatsoever arising out of any accident or injury which may be caused by or results from their participation while engaging in activities at or sponsored by any of the City of Elliot Lake facilities, parks, or fields. The Corporation of the City of Elliot Lake and its staff, Council, volunteers, trainers, instructors, agents, and representatives (Collectively the “City”), as well as its affiliated Provincial Associations and member clubs (Collectively the “Affiliates”), are not responsible for any injury, death, damage, expense, loss of income or loss of any kind suffered by a participant during, or as a result of, the activities, or caused by the risks, dangers and hazards associated with the activities.

I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations while participating in swimming lessons. I confirm that I have read and fully understand this waiver and release of liability. I sign this waiver and release of liability voluntarily without any inducement, assurance, or warranty being made to me.

Refund/Cancellation Policy 

Missed classes cannot be rescheduled. We understand that life can be unpredictable, but we ask that you notify us in advance if you are unable to attend. Refunds are only available due to medical reasons, and a doctor's note will be required to process any refund requests.
