Mobile Playground Truck
rental application and waiver

I understand that if I also want a shelter booked at a park, I need to call the Lakeside Rec Facility office M-F 9am-5pm to check availability and pay.

Please note the following rules/notes for renting the Mobile Playground Truck: 

1. 2 hour minimum rental period. Vehicle will arrive and stay parked at one location, and within Galesburg city limits. 

2. No food or drinks allowed near equipment.  

3. No patrons allowed inside or on the vehicle.  

4. Vehicle cannot be parked on private property. Must be parked on public property.  

5. All patrons must be respectful of the equipment and vehicle. This includes following any rules, listening to onsite staff, picking up after any messes made, helping put away all equipment, and behaving in a safe  respectable manner.  

 6. All items taken off of the truck must be put back on the truck by the end of the rental time.  

7. All COVID protocols will be followed at time of rental. As of 1/25/2023, there are no COVID restrictions.

8. Mobile Playground Truck dates are available between Memorial Day-Labor Day weekends.

It is hereby understood that the undersigned applicant will arrive at opening time, remaining until closing time,  be completely responsible for the proper conduct of the group, and leave the equipment and truck in a clean, working manner. The rental time includes ALL TIME with the vehicle.

Cancellations must be communicated to the office no later than 24 hours prior to the event to be eligible for a refund. This communication  must be done during business hours by calling 309-345-3683. (If an event is on Sunday and needs to be cancelled, the office would need notification by 5:00 PM on the preceding Friday).  Refund amount is amount paid minus a 10% administrative fee. Weather cancellation by patron (less than 24 hours in advance): an account credit partial refund can be issued, prorated for the amount of time the truck was there, including travel, any set up/clean up time, and 10% administrative fee. 

I agree to waive and relinquish all claims my party or I may have as a result of renting the truck and equipment and participating,  against the City of Galesburg and their officers, agents, servants, and employees. I do hereby fully release and discharge the City of Galesburg and their officers, agents, servants and employees from any and all claims from  injuries, damage or loss, which may have accrued to our rental party and arising out of, connected with, or in  anyway, associated with the activities of the rental party.  

If damages occur to the facility or equipment, I understand that I may be charged for repairs or replacements. 

I  agree to allow photos to be taken and used by the City of Galesburg for marketing purposes.
