Hockey Manitoba MHA Confidentiality Agreement for HCR Users

Please read the following information carefully and answer any questions that may be asked.

As a staff and/or volunteer member of Hockey Manitoba, I understand that I may be provided with access to personal data through the course of my work and/or association with Hockey Manitoba. This data may include, but is not limited to, personal information about other Hockey Manitoba staff and/or volunteer members, players and prospective members and players.

I have carefully read, agree with, and accept all of the terms and conditions contained in the Hockey Manitoba Privacy Policy. In understand that my Hockey Canada Registry Account is for my own use only, and not to be shared with any other individuals. There is zero-tolerance for shared accounts and any users being found to have done so will lose their HCR privileges immediately and permanently. 

I understand that by accepting the Privacy Policy, I expressly consent to manage all personal information that is collected, used, kept, secured, and disclosed on behalf of Hockey Manitoba in accordance with the terms of the Privacy Policy.

I understand that Hockey Manitoba reserves the right to amend the Privacy Policy at any time by posting the amended terms on the Hockey Manitoba website (www.hockeymanitoba.ca).   All amended terms shall automatically be effective 30 days after they are initially posted on the website. Members of Hockey Manitoba must check the Privacy Policy periodically to inform themselves of any changes. I agree that my continued employment and/or affiliation as a staff and/or volunteer member of Hockey Manitoba will be considered acceptance of any amendments to the Privacy Policy.

Please note that once the form is completed it will take up to 4 business days to receive your HCR access information. Access will only be granted if all relevant forms have been completed by the appropriate member(s) of your MHA and only if there are no outstanding fees.

I acknowledge that I have been given an opportunity to prevent or correct any error in connection with this information. After submitting this form, if I subsequently need to make any changes it is my responsibility to contact the sender of this form. I understand and acknowledge that if I proceed to sign this document electronically that the document will be valid and enforced in the same manner as a hand-signed document that exists in physical form and that a record or signature may not be denied legal effect or enforceability under law solely because it is in electronic form. I have read, understood and agree to Activity Messenger's Terms of Service and Privacy Policy and I consent to Activity Messenger storing this form. I understand, agree to and acknowledge the previous paragraph.
