March Break Camp – Active Sports: High Five Camp

Assumption of Risk, Consent & Liability

In consideration of my child’s participation in the March Break Active Sports – High Five Camp offered by Busy Minds Education®, I, the undersigned parent or legal guardian, hereby agree to the following terms and conditions:

Acknowledgment of Risks

I acknowledge that participation in the March Break Active Sports – High Five Camp involves a variety of activities, including but not limited to non-competitive sports, basic sports skill development, cooperative games, movement-based activities, and physical play. I understand that while every effort is made to maintain a safe, supportive, and age-appropriate environment, there is always a possibility of injury, allergic reaction, or unforeseen circumstances.

Medical Clearance

I certify that my child is in good physical and mental health and is capable of participating in the March Break Active Sports – High Five Camp. If my child has any medical conditions, allergies, physical limitations, sensory sensitivities, or special considerations, I have disclosed this information and obtained appropriate medical clearance if required.

Supervision

I understand that my child will be supervised by qualified Busy Minds Education® instructors and staff during the March Break Active Sports – High Five Camp. I agree to inform staff of any relevant information that may affect my child’s participation, safety, or well-being.

Code of Conduct & Dismissal Policy

I acknowledge that all campers are expected to follow camp guidelines related to respect, safety, teamwork, and appropriate behavior. Any child who engages in inappropriate behavior—including but not limited to physical aggression, bullying, unsafe conduct, or repeated disruption—may be dismissed from the program without refund, at the discretion of Busy Minds Education®, to ensure the safety and well-being of all participants and staff.

Release of Liability

In consideration of my child’s participation in the March Break Active Sports – High Five Camp, I hereby release, discharge, and hold harmless Busy Minds Education®, its instructors, employees, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising from injury, illness, loss, or damage that may occur during participation in the program.

Emergency Medical Care

I authorize Busy Minds Education® and its representatives to seek emergency medical care for my child in the event of illness or injury. I understand that reasonable efforts will be made to contact me or the emergency contact listed; however, I consent to necessary medical treatment if deemed required by medical professionals.

Photography & Media Consent

I grant permission for Busy Minds Education® to use photographs or videos of my child taken during the program for promotional and marketing purposes. If I do not wish for my child to be included, I understand that I must notify Busy Minds Education® in writing by emailing ottawa@busyminds.ca.

By signing below, I confirm that I have read, understand, and voluntarily agree to the terms outlined in this Assumption of Risk, Consent, and Liability Agreement. I further confirm that I am the parent or legal guardian of the participating child.
