By signing below, I, the undersigned participant or parent/guardian of the participant, hereby
grant Whistler Gymnastics and its representatives, employees, and volunteers the irrevocable
right and permission to use, publish, and reproduce photographs, videos, and/or other media
(collectively, "media") taken of me or my minor child during participation in gymnastics classes,
events, and other related activities.
Purpose of Use: I understand that this media may be used for a variety of purposes, including
but not limited to:
● Promotional materials, such as flyers, brochures, and advertisements.
● Whistler Gymnastics' official website and social media channels (e.g., Instagram,
Facebook, YouTube).
● Educational or training purposes within the club.
● News and media releases.

Electronic signature*

Release and Waiver: I hereby release and discharge Whistler Gymnastics, its employees,
representatives, and agents from any and all claims, demands, or causes of action arising out of
or in connection with the use of the media, including any claims for privacy, defamation, or right
of publicity.
I understand that I will receive no compensation for the use of the media. All media created and
used for these purposes shall be the sole property of Whistler Gymnastics.
Right to Opt-Out: I understand that if I or my child do not wish to be included in any media, it is
our responsibility to notify the photographer or instructor at the time the media is being taken.
However, I acknowledge that in group settings, it may not be possible to fully avoid inclusion.
Declaration: I have read and fully understand this form. By signing, I agree to the terms and
conditions outlined above.
