I, the participant, or parent / legal guardian of the designated above-named participant, acknowledge that participation in dance classes, rehearsals, performances, and related activities at Aria Dance Academy LLC involves inherent risks, including but not limited to physical and emotional injury, falls, sprains, strains, and other potential hazards. I recognize that severe injuries can occur in any activity involving height or motion, including dancing, stretching, conditioning, tumbling, and physical activity in general.

I understand that it is the expressed intent of all Aria Dance Academy LLC staff and personnel to provide for the safety and protection of myself or the above-named participant(s) and in consideration for using these facilities, or allowing the above-named participant(s) to use these facilities, I hereby voluntarily COVENANT NOT TO SUE and FOREVER RELEASE Aria Dance Academy LLC, this facility, affiliated and partner companies and organizations, property owners and lessors, staff, contractors,
subcontractors, teachers, coaches, owners, directors and other members involved in this facility's program(s), from all liability and for any and all damages and injuries suffered by myself or the above-named participant(s) during instruction, supervision, and/or control during any and all classes or extra activities, whether caused by negligence or otherwise.

MEDICAL AUTHORIZATION: In the event of an injury or medical emergency, I authorize Aria Dance Academy LLC and its staff to seek medical treatment for myself or the above-named participant(s) at my expense. I understand that I am responsible for any medical costs incurred as a result of participation.

“P a y m e n t T e r m s & C o n d i t i o n s”

There will be 36 weeks of instruction and classes throughout the 2026/27 year, divided into 3 equal payment terms. Due to statutory holidays, Monday dancers will only receive 34 classes, and prices have been adjusted to reflect missed Mondays.
To secure a spot in a class, clients must provide a credit card number which will be safely stored through a third-party billing provider. All future installments are auto charged to the stored credit card number on file.
Clients Pre-Registering will be invoiced and charged a non-refundable deposit of $25/class at the time of enrollment. The remaining balance for Term 1 Payments will be auto charged on June 1, 2026.
Clients Registering after Jun. 1, 2026 and before Aug. 10, 2026 will be invoiced and charged a non-refundable deposit of $25/class at the time of enrollment. The remaining balance for Term 1 Payments will be auto charged on Aug. 10, 2026.
Clients Registering after Aug. 10, 2026 and before Aug. 24, 2026 will be invoiced and charged the full amount Term 1. Tuition paid after August 10, 2026 is non-refundable.
Clients Registering after Aug. 24, 2026 will be charged a non-refundable prorated tuition amount to reflect missed classes in that payment term.
If a student wishes to withdraw from a class, Aria Dance Academy must receive written notice 14 days before the next term begins: by Aug. 10, 2026 for Term 1 – by Nov. 2, 2026 for Term 2 – by Feb. 15, 2027 for Term 3. If written notice is received, cards will not be auto charged. Exceptions may only be made to Aria Dance Academy’s refund policy if a doctor’s note has been received.
If a student is switching to another dance class, program transfers can be accommodated as long as the move is made before Jan. 1, 2027, and providing there is space available in the new class.
An $85 + tax /class recital costume fee is due and auto charged on Jan. 1, 2027. If a dancer will not be participating in the recital, Aria Dance Academy must be notified by Jan. 1, 2027. If notice is received, cards will not be auto charged for costumes.
For insurance purposes, it is mandatory for parents and/or guardians to sign and agree to the Aria Dance Academy “Release of Liability” included online. Dancers will not be allowed to participate in class until this form has been signed and returned.
