VAULT DANCE STUDIO LIABILITY RELEASE AND WAIVER AGREEMENT

This Liability Release and Waiver Agreement ("Agreement") is executed by the undersigned individual (or the parent/guardian of a minor participant) in favor of Vault Dance Studio its owners, directors, officers, employees, instructors, contractors, agents, and representatives (collectively, the "Studio").

In consideration for being allowed to participate in any dance classes, activities, events, rehearsals, or other programs offered by the Studio, the undersigned agrees to the following:

1. Assumption of Risk

I acknowledge and understand that participation in ballroom dance classes, rehearsals, performances, competitions, and other activities at the Studio involves inherent risks, including but not limited to, physical exertion, slips, falls, collisions, and injuries. I voluntarily choose to participate (or allow my minor child to participate) with full knowledge and understanding of these risks.

I, for myself (or on behalf of my minor child), assume all risks associated with participation in any and all activities offered by the Studio, whether caused by the negligence of the Studio or otherwise.

2. Waiver of Liability

I hereby release, waive, discharge, and covenant not to sue the Studio for any and all claims, demands, or causes of action arising out of or related to any loss, damage, injury, or death sustained by me or my minor child in connection with participation in the Studio’s activities, including but not limited to:

• Dance instruction,
• Studio events,
• Use of Studio premises and equipment,
• Participation in competitions, performances, or off-site events sponsored or affiliated with the Studio.

This waiver applies regardless of whether the injury or harm is caused by the negligence of the Studio.

3. Medical Treatment

I authorize the Studio to administer or seek emergency medical treatment for me (or my minor child) in the event of an injury or illness during any Studio activities. I agree to be solely responsible for all costs and expenses incurred in connection with such medical treatment.

4. Indemnification

I agree to indemnify and hold harmless the Studio from any and all claims, actions, damages, costs, and expenses (including attorney's fees) arising from any injury, harm, or damage caused by me (or my minor child) to others or property while participating in Studio activities.

5. Photographic and Video Release

I grant the Studio the right to use photographs and videos taken of me (or my minor child) during classes, rehearsals, performances, competitions, or other Studio events for promotional and marketing purposes, including but not limited to social media, websites, brochures, and advertisements, without further compensation or approval.

6. Parent/Guardian Consent for Minor Participation

If the participant is a minor, I, as the parent or legal guardian of the minor, have read and understood this Agreement and agree to its terms on behalf of my minor child. I further agree to ensure my child’s compliance with all Studio rules and policies.

7. Governing Law and Severability

This Agreement shall be governed by and construed in accordance with the laws of the State of Oregon, without regard to its conflict of law’s provisions. If any part of this Agreement is deemed invalid or unenforceable, the remaining provisions shall continue in full force and effect.

8. Acknowledgment of Understanding

I have read this Agreement, fully understand its terms, and sign it voluntarily with full knowledge of its legal significance. I understand that by signing this Agreement, I am giving up substantial rights, including the right to sue the Studio.
