2 Day Per Week Membership – Includes two instructor-led ninja classes per week. Select your class days/times after membership purchase. This membership is for one individual and cannot be shared between family members. This is a recurring membership that automatically renews every 30 days until canceled.

Unlimited Membership – Includes unlimited ninja classes and open gyms each month. Classes must be within the participant's designated age group. Unlimited Membership is for one individual and cannot be shared between family members. This is a recurring membership that automatically renews every 30 days until canceled.

Each membership will renew monthly until cancelled. When purchasing a membership you will be asked to select the day or days of the week you will attend classes

I AM SIGNING FOR MYSELF OR ON BEHALF OF MY CHILD AND I AM AT LEAST 18 YEARS OLD, AND AGREE THAT THE TERMS BELOW ARE CONTRACTUAL. MY INTENT IN SIGNING THIS DOCUMENT IS TO ACKNOWLEDGE AND ASSUME THE RISKS INVOLVED IN THIS UNDERTAKING AND TO RELEASE NEX LEVEL NINJA, LLC, NEX LEVEL ARENA LLC, ITS AGENTS AND ASSIGNS, FROM ANY LIABILITY NOT CAUSED BY ITS DIRECT AND WILLFUL NEGLIGENCE WITH RESPECT TO MY INVOLVEMENT, INJURY OR DEATH IN THIS ACTIVITY. I APPROVE OF MY CHILD’S ATTENDANCE AT THIS ACTIVITY AND CERTIFY THAT S/HE IS IN GOOD HEALTH AND IS FIT TO PARTICIPATE. I UNDERSTAND THAT THERE ARE INHERENT RISKS IN THIS ACTIVITY, WHICH HAVE BEEN CONSIDERED AND WHICH THE PARTICIPANT ASSUMES. PARTICIPANT HAS MEDICAL INSURANCE. I AGREE TO HOLD HARMLESS NEX LEVEL NINJA L.L.C/NEX LEVEL ARENA LLC AND THEIR AGENTS FROM CLAIMS OR DAMAGES DUE TO INJURY TO PERSON OR PROPERTY ARISING FROM MY CHILD’S PARTICIPATION IN THIS ACTIVITY. I CONSENT TO EMERGENCY TREATMENT FOR MY CHILD, IF IN THE JUDGEMENT OF NEX LEVEL NINJA LLC, NEX LEVEL ARENA LLC IT IS REQUIRED.

I hereby authorize the NEX LEVEL NINJA LLC/NEX LEVEL ARENA LLC to allow the reproduction, dissemination and/or publication of my name and likeness for media coverage, public relations, or any other purpose which may involve the use of photographs, films, and/or video tape recording. This is to be done in conjunctions with my participating in NEX LEVEL NINJA AND NEX LEVEL ARENA events and I understand and agree that I may neither pay a fee to receive individual promotional consideration from my participation in events, nor will I receive any payment for the possible commercial use of my name or likeness.

THIS WAIVER HAS BEEN READ AND UNDERSTOOD AND IS SIGNED VOLUNTARILY AS THE LEGAL REPRESENTATIVE FOR THE PARTICIPANT
