RHEAL Program Report and Invoice

This form is used to 1) submit your invoice and 2) report on RHEAL Programs and is a requirement of the RHEAL Program contract. 

Include information about the program, such as a brief description, number of participants, number of sessions, etc. Write accurately and clearly as reports are shared with board members, funders, etc. 

Reports and invoices must be submitted by March 31st or the end of the program, whichever comes first.
