This form is to be completed by candidates who do not have a current NCCP coach/instructor that is able to evaluate their rider levels.

Additional items candidate must complete:

MHC liability waiver (Adult Waiver or Youth Waiver)
Administration fee of $30 per certificate issued for minors or $31.50 per certificate issued for adults, payment accepted upon completion of this form.

Once we receive the above we will contact you with a list of evaluators for you to contact.
