Permission and Waiver

On behalf of my child, I understand that my child’s participation in Camp Ak-O-Mak for Girls program is completely voluntary

• I have familiarized myself with Camp Ak-O-Mak’s program and activities. 
• I give permission for my child to participate in any activities on the camp premises and any trips out of camp during the summer. 
• Although Camp Ak-O-Mak takes reasonable safety measures to minimize the risk of injury, I recognize that certain risks are imminent in the camping industry. I therefore release Camp Ak-O-Mak from and waive any claim for injuries, loss, or other damages while on or off camp property during my child’s stay at camp. 
• I further recognize and have instructed my child in the importance of knowing and abiding by Camp Ak-O-Mak’s rules, regulations and procedures for the safety of participants. 
• I also give permission for my daughter’s likeness to be used in camp promotional materials such as, but limited to, Camp brochure, website, video, social media posts. 
• I also agree and accept to pay any and all damages should my child participate in vandalism. 
• I agree and accept that the proper forum for ANY legal action against Camp Ak-O-Mak or it’s associated personnel, including Medical, is the Province of Ontario, Dominion of Canada. 
• I understand/accept that Store Purchases, Out of Camp Events with Food Purchases & Travel, additional Medical Expenses will be charged to my child's Refundable Expense account. An accounting of the expense account will be done at the end of summer. An invoice will be issued for any additional expenses payable online. 
• I understand and accept Camp Ak-O-Mak will be contacting by email.

Terms and Conditions

• You will receive email acknowledgement within 48 hours of receipt of Registration form during business hours. 
• A valid credit card is required to reconcile charges in excess of refundable expense account which may include elective events / activities, associated travel / food, medical expenses, camp store; Airport travel, Rentals and extra items if used. Final processing of the Refundable Expense Account will be completed after camp and a statement of accounting emailed to the parent. Please ensure the camp has a valid email address. 
• Refund Policy: Refund (less $750.00 non-refundable deposit) for cancellation by May 1st, 2026. Full refund may be available only for a medical reason with a doctor's note or registration may be transferred to next season. No credit is given for late arrival or early withdrawal. Any refund given will be charged a 5% admin fee if you have paid by credit card. 
• Camp is not responsible for lost or stolen property. Avoid sending expensive items and please label all the campers belongings. 
• Referral Credit—Families referring a new family that signs up a camper will receive a $100. credit on campers Refundable Expense Account. 
• Camp Ak-O-Mak for Girls maintains a code of conduct for the campers and reserves the right to assess a Camper’s behavior, to revise a Camper stay, or to dismiss a Camper early, when it is deemed necessary in the interest of the child and Camp Ak-O-Mak for Girls, No refunds shall be given in these cases. Parents agree to provide full information on Campers in advance. 
• I agree Camp Ak-O-Mak is released from liability in connection with medical treatment and unavoidable accidents. Camp Ak-O-Mak has my permission to use necessary medical measures in the event of an emergency. 
• My child has permission to leave the campgrounds with authorized camp staff for scheduled trips and outings. I hereby agree that if I commence any legal proceedings, they will be held only in the Province of Ontario, and I hereby irrevocably submit to the exclusive jurisdiction of the Courts of Ontario. 
• Campers vaccinations should be up to date. 
• Any additional sporting or out trips to our programming will increase the price of your store account
