Assumption Of Risk, Consent & Liability
I hereby expressly and affirmatively state that I have voluntarily submitted participation with the Tennis Connection and CourtSports. I certify and acknowledge that my participation in any activity involves a risk of injury that are a common and ordinary occurrence with such activities.
I hereby freely and expressly ASSUME and accept ANY AND ALL RISKS to me or any member of my family while participating in the activities. Further, I voluntarily elect to participate, or have my child participate, in the activities. As consideration of engaging in the activities, I agree to ASSUME ALL RISKS associated with the activities and agree to hold harmless, release, defend and indemnify the Tennis Connection and CourtSports and its officers, agents and employees, from any and all claims and causes of action, including those for injury or death to persons or damage to property arising from participation in the activity.
I hereby agree to indemnify and hold harmless the Tennis Connection and CourtSports for any injuries to me or to other person(s) or property that I may cause because of participating in the activities. I further waive, release, and forever discharge the Tennis Connection and CourtSports and all of their representatives, and all others from any and all responsibilities or liability for injuries or damages resulting from participation in the activities. This Agreement and Release of Liability applies to the undersigned any children of the undersigned participating in the activities.
I further certify that the participant is in good health.
WARNING: THIS WAIVER AND RELEASE ACTS AS A RELEASE OF YOUR RIGHTS. BY EXECUTION OF THIS AGREEMENT, YOU ARE AFFIRMING THAT YOU HAVE READ THIS DOCUMENT AND AGREE TO THE CONDITIONS STATED HEREIN. IF, YOU DO NOT UNDERSTAND THIS AGREEMENT, YOU MAY WISH TO CONSULT AN ATTORNEY OF YOUR CHOICE.

I understand that my photograph, and/or my child’s (person participating in the activities) photo may be taken during his/her participation in any group activities, and I give permission to display said photos, if taken.
