This form is exclusively for parents who have purchased gift certificates (pre-paid weeks) during the Gift of Camp promotion for $200. Previously purchased Gift Certificates are the only available option for payment on this form. The regular form must be used to purchase additional weeks at the regular price. Registration must be done one child at a time.
