This form must be filled out and submitted for all activities that will occur outside of Manitoba. Activities include, but are not limited to, competitions, training camps, clinics, etc. An authorization form is required for each activity and each club. 

Minimum Requirements:

• All participants must be registered with the MGA and have completed all registration requirements for their respective role.
• Coaches and Managers must be a legal age (minimum 18). 
• This form and payment must be received a minimum of 60 days from the first day of the event/activity.  If the MGA is not made aware of travel to an event/activity no insurance coverage will be provided. 

Note 1: MGA's sport accident insurance only incidents that may occur during the course of the approved event/activity while participating on the field of play. It does NOT include incidents that may occur off the field of play, or during non-training and/ or competition activities such as but not limited to a social event and travel to and from the venue. 

Note 2: MGA's sport accident insurance policy does NOT provide coverage for US or International medical expenses, it only provides coverage for Canadian based medical expenses. Participants are STRONGLY encouraged to obtain their own travel medical policies that do not have sport exclusions. Note 3: For International competitions (non-US) an additional application process may be administered via Gymcan however their process is still under discussion so please email mga.kmiller@sportmanitoba.ca for more information.
