For my child to participate in the Ringette Manitoba Ring Skate, and in doing so I understand that I am assuming responsibility for all risk inherent to the sport. I authorize Ringette Manitoba to use and to disclose any of the personal information subject to the law for the purpose of registering my child with Ringette Manitoba and communicating with me for ringette purpose only. I authorized the use of any video or photographic image or likeness to be used for general marketing and communication process of the sport.
