Student Participation Agreement

All students must have a signed copy of the Student Participation Agreement on file in order to participate in PHAME classes and programs.

Code of Conduct

Media Consent
I give PHAME permission to use my name, voice, image, words, and/or artwork—captured during classes, workshops, rehearsals, performances, and other PHAME-related events or programming—in any media (including print, social, online, radio, television, and film). This may be for the purpose of promoting PHAME’s mission, sharing its activities, applying for funding, or other uses as PHAME sees fit. I understand that I will not receive compensation for this use. If I want to revoke this permission, I will notify PHAME in writing at info@phamepdx.org. Until I send such notice, my permission will remain effective.
Program Hours
I understand that PHAME’s campus (the building(s) where programming is held) officially opens 30 minutes before programming starts and closes 30 minutes after it ends, and PHAME has no duty with respect to my safety outside of these times. I agree not to enter the building more than 30 minutes before programming starts and to leave promptly and no later than 30 minutes after it ends.
Arrival, Departure, & Transportation
I will plan my travel so that I arrive no earlier than 30 minutes before programming begins and leave (or get picked up) within 30 minutes of programming ending. PHAME staff will stay with me until I leave, but I understand that arriving too early or staying too late will cause strain on staff and the program. I understand that I am responsible for arranging my own transportation to and from all PHAME classes, rehearsals, events, and other programming. PHAME staff, teachers, volunteers, and board members will not provide or arrange transportation.
Late Ride Policy
If I know my ride will arrive more than 30 minutes after programming ends, I will notify PHAME staff as soon as possible. I understand that if my ride is running severely late (arriving more than an hour after programming ends), PHAME reserves the right to call me a cab/Uber. I understand that I will be charged for this ride.
Open Campus Responsibility
PHAME is an open campus, which means I am free to leave during the school day, and from rehearsals, performances, events, and other programming. I understand PHAME is not responsible for ensuring I stay on campus during these times, and PHAME has no duty with respect to my safety once I leave the PHAME campus. If I need help staying on campus, I will bring a support person with me to PHAME.
Non-PHAME Events
If I choose to attend a non-PHAME event or activity, even one recommended by PHAME, I understand that I do so at my own risk. I agree that PHAME is not responsible for anything that happens at such events.
Liability Waiver
I hereby release and agree to hold harmless PHAME, its staff, board members, volunteers, and partners (each referred to as a “Released Party”) from any claims, liabilities, or legal issues that may arise through participation in PHAME activities, including without limitation claims related to physical injuries, actions of other students, and Covid and other communicable diseases, except to the extent solely caused by the negligence of a Released Party and covered by liability insurance available to such Released Party. With respect to any such covered-claim, I waive the right to recover damages that exceed the amount of liability insurance available to the Released Party. I further waive any claim for consequential or punitive damages in any amount.
Emergency Medical Consent
In case of a medical emergency, I give PHAME permission to seek medical care as needed. I understand I am responsible for any related costs.
Guardian Signature (If Applicable)
If I have a court-appointed legal guardian, I understand this form must also be signed by my guardian to be valid.
