I, the undersigned parent, give permission to the Town of Banff and/or Settlement Services in the Bow Valley to photograph/video my under-18 child/youth and use such photograph(s) /video(s) /recording(s) in all forms of media including advertising, display, audiovisual, Internet/online, exhibition or editorial use for use in connection with the activities of the Town of Banff or for promoting, publicizing, or explaining the Town of Banff or its activities.

The Town will not release the name of the child/youth in connection with the images.

I understand and agree that I will not receive any payment for the child/youth time or expenses or any royalty for the publication of the photographs(s)/videos(s)/recording(s), and I hereby release Settlement Services in the Bow Valley-Town of Banff from any such claims.

I certify that I have read and fully understand the above consent and release, and that all questions pertaining to this consent have been answered to my satisfaction.
