Direct Deposit / EFT Authorization Form

Gymnastics BC (GymBC) is moving towards electronic payments via Electronic Funds Transfer (EFT) instead of physical cheques, though there may still be some instances where cheques are processed instead of EFTs. In order to be eligible to receive electronic payments from GymBC, you need to submit the following EFT authorization form.
