2025 Canada Summer Games Athlete Identification Form

The 2025 Canada Summer Games will be held from August 8-24, 2025 in St. John’s Newfoundland and Labrador.

Please complete this form to indicate your interest in being considered for the 2025 Canada Summer Games indoor team.

In order to be eligible athletes must be 18 and under (18U) as of December 31, 2025 (Year of birth: 2007 or later).

This form is the final opportunity for athletes to express their interest. If you were previously identified during the 2024 Team Manitoba programming we ask that you complete this form as well to ensure we have your current information on file.
