The law requires individuals to provide their Social Insurance Number (SIN) to any person/organization that must produce a Relevé 24 in their name (cf. https://www.revenuquebec.ca/en/).

In order to issue your Relevé 24, we therefore need to collect the following information.

• Social Insurance Number (SIN)
• Mailing address
• Email address

No statement will be issued if the information is not on file.
