By signing this waiver, I agree and understand the following on behalf of my child under the age of 18.

My child is participating in the yoga classes offered by Yoga in Fergus during which I will receive information and instruction about yoga. I recognize that yoga requires physical exertion which may be strenuous and may cause or aggravate physical injury and I am fully aware of and willingly assume the risk and hazards involved. As is the case with any strenuous exercise, I understand that there is a possibility of injury and that it is my responsibility to consult a physician regarding my ability to participate in yoga practice prior to my class at Yoga in Fergus. I affirm that it is my responsibility to advise every instructor at Yoga in Fergus of any physical conditions, which may limit my participation in the yoga studio. Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga practice and/or specific poses are not recommended for individuals with certain medical conditions.

I am responsible for all personal belongings. Yoga in Fergus will not be responsible for lost or stolen belongings.

I release and discharge Yoga in Fergus, its owners, directors, staff, participants and its instructors from any and all liability, claims, demands, or actions that I may have resulting from injury, death or damages arising from my participation in the yoga classes at any Yoga in Fergus. I have read and recognize that the above release and waiver of liability is a legal contract and fully understand its contents. I agree to the terms and conditions above.

*For "Youth", "Kids" and "Family" yoga classes:

I, as parent or guardian of the minor child participating in class, am aware that participation in a sport or physical exercise, such as yoga, may result in accident or injury, and hereby assume the risk connected with any participation from myself or my child in any yoga class with Yoga in Fergus.

I specifically agree that Yoga in Fergus shall not be liable for any claim, demand, cause of action of any kind whatsoever for, or on account of, any kind of personal injury, property damage or loss of any kind resulting from or related to my child’s use of the facilities or participation in any exercise or activity at Yoga in Fergus, and I agree to hold Yoga in Fergus harmless from same.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above. here
