The purpose of this form is to notify the Manitoba Games of any athlete who intends to travel separate from their team to / or from Thompson, Manitoba with a parent / guardian.

For example:

An athlete who plans to remain in Thompson, Manitoba with a parent / guardian after completion of the Manitoba Games.
An athlete who plans to leave Thompson, Manitoba with a parent / guardian prior to completion of the Manitoba Games.
An athlete who plans to travel to / from Thompson, Manitoba with a parent / guardian instead of using the Manitoba Games provided bus transportation.
