Liability Waiver for Kids Yoga Class

I, [Parent/Guardian], hereby acknowledge and agree to the following terms and conditions in consideration of my child participating in the Kids Yoga Class provided by Busy Minds Ed.

Assumption of Risk: I understand that participating in any physical activity, including yoga, involves inherent risks and dangers. I am aware that these risks may include but are not limited to physical injury, illness, or even death. I voluntarily assume all such risks on behalf of my child and myself.

Health and Medical Condition: I certify that my child is physically fit and healthy to participate in the Kids Yoga Class. If my child has any known medical conditions or injuries, it is my responsibility to inform the instructor prior to the start of each class.

Parental Responsibility: I understand that it is my responsibility as a parent/guardian to ensure that my child follows all instructions given by the yoga instructor during the class. I will instruct my child to refrain from engaging in any activities that they feel are beyond their physical capabilities or comfort level.

Release of Liability: In consideration for allowing my child to participate in the Kids Yoga Class, I hereby release, waive, discharge, and covenant not to sue Busy Minds Ed., its owners, instructors, employees, volunteers, or any other individuals associated with the organization from any and all liability for any claims or damages arising out of or resulting from participation in this class.

Indemnification: I agree to indemnify and hold harmless Busy Minds Ed. from any loss, liability, damage or costs (including attorney fees) incurred due to my child's participation in the Kids Yoga Class.

Severability: If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall continue to be binding and enforceable.

I have read this Liability Waiver for Kids Yoga Class carefully and fully understand its contents. I am aware that by signing this document, I am waiving certain legal rights on behalf of myself and my child. I voluntarily agree to all terms and conditions stated herein.
