Waiver, Release of Liability, and Consent Form

I want to participate in events organized or sponsored by Canadian Parents for French MB, the Fédération des parents de la francophonie manitobaine, the Notre Dame Recreational Centre, and Sports en français, including the event mentioned above. In exchange for permission to participate and use any equipment provided during these events, I agree to the following:

Risks

My participation could lead to injury/death or damage/destruction of property. I am fully and solely responsible for my safety while participating in the events, and I accept and assume all risks, known and unknown, associated with my participation in the events.

I understand that the instructions given through the classes are for informational purposes only. I understand that if all necessary precautions are taken by the instructor, he/she cannot be held responsible for my incorrect practice at any time. To ensure that no injury occurs, I agree to adapt my participation according to my limits. I declare that I take full responsibility for myself during the activity. 

Physical Condition

I am physically fit to participate in the events, and I have no medical condition that would impair my ability to participate in the events or endanger any other participant in the events.

Waiver, Release and Commitment Not to Sue

I release and discharge Canadian Parents for French MB, the Fédération des parents de la francophonie manitobaine, the Notre Dame Recreational Centre, and Sports en français, and their administrators, officers, employees, volunteers, sponsors, and representatives from any damage, claim, injury, liability and cost (the "Claims") related to my participation in the events and use of equipment, including all Claims arising from negligence by Canadian Parents for French MB, the Fédération des parents de la francophonie manitobaine, the Notre Dame Recreational Centre, or Sports en français, or any other individual mentioned above, and I agree not to sue either Canadian Parents for French MB, the Fédération des parents de la francophonie manitobaine, the Notre Dame Recreational Centre, or Sports en français or any individual mentioned above for any of these Claims.

Release of Liability

I will indemnify and hold harmless Canadian Parents for French MB, the Fédération des parents de la francophonie manitobaine, the Notre Dame Recreational Centre, and Sports en français, and the individuals mentioned below from any liability for all Claims of whatever nature resulting from or related to my participation in the events or use of equipment, including any damage or harm that I may cause to a third party or their property, as well as any damage or harm that may be caused to me or my property by a third party.

Photos and Videos

I grant my consent to Canadian Parents for French MB, the Fédération des parents de la francophonie manitobaine, the Notre Dame Recreational Centre, and Sports en français, and their representatives and contractors to take photos and videos of me during the events and modify, publish or otherwise use said photos and videos in any manner whatsoever including compilations, publications, catalogs, posters, banners on websites, advertisements for products and services as well as for other commercial purposes. I waive any rights I may have over the photos and videos including any right to inspect or approve these photos or videos and their use. I also acknowledge that I will receive no compensation regardless of how my photos/videos/my name/my image are used.

Miscellaneous

I have reached the age of majority in the province where the events are offered. I understand that this acceptance applies to all activities/events/programs supported/sponsored/and/or conducted by Canadian Parents for French MB, the Fédération des parents de la francophonie manitobaine, the Notre Dame Recreational Centre, and Sports en français. This agreement binds me as well as my executors/adminstrators/and representatives. I have had an opportunity to seek legal advice before signing this document; however; I voluntarily chose not to do so.
