I have read the Elk Valley Gymnastics Competitive Team Program Handbook.  I fully understand the responsibilities and privileges of the membership for the parents and gymnasts.  I also acknowledge that this agreement and my acceptance will continue through the duration of my child’s participation in the TEAM.  I also agree that my payments will be kept up to date and that failure to do so may impact my child’s training at the gym.

I understand that by signing below, I accept responsibility for the total financial obligation for all expenses incurred for my gymnast for each and every year my child participates in the competitive program and I understand that each year I will need to Re-sign a contract in order to keep my child’s spot on the team. I will allow Elk Valley Gymnastics to charge my card as needed:

 Monthly training  (1st of the month)
 Competitions I have signed up for ( 15th of the month) 
Bodysuit and team wear ( as needed) 
Camps ( first day of camps) 
 1 book of Raffle Tickets 

 I understand that as an EVGC Comp Parent I have the obligation to volunteer over the course of the year. 
I will sell/purchase 1 book of raffle tickets 
I will participate in one bottle drive during the year 
In total, I will volunteer 8 hours over the course of the Sept - June 2022/2023 year not including the Rocky Mountain tumble 

I have attached my postdated cheque to this form for $350. If I do not complete the designated hours my $350 cheque will be cashed on June 30^th 2023

I acknowledge that during the  Rocky Mountain Tumble  I will volunteer for 1 set up or take down shift and a minimum of 1 shift during the event

I understand that monthly payments will to made to EVGC by either credit card, e-transfer or post-dated cheque or cash. Failure to make payments for more than 3 months will affect my child’s ability to train and complete until payment can be made. 

I understand that all communication from Elk Valley Gymnastics is done through emails, I will keep up to date and respond accordingly to any and all emails sent out that apply to the competitive program.
