Consent for YFNED Spring Break Ross River Programming

YFNED is organizing different activities for children and youth of all ages around both the Ross River School and the Yukon University building between March 9 - 13 and March 17 - 19.

This permission is for any of the activities taking place on those dates.

Activities in the first week will be centred around fine and gross motor skills, as well as arts and wellness.

Activities in the second week will all have to do with science and math but in a fun and hands-on way, working with cardboard, computers, and other general craft supplies.

Consent for release of personal information to Indigenous Services Canada

Funding for YFNED programs and services is under threat from Jordan’s Principle (Indigenous Services Canada). In order to meet Canada’s new requirements, we need to collect information about all the Indigenous kids who might access our services.

Will your child wish to access YFNED programs like: school food, trades workshops, numeracy camps, cultural activities, mobile therapeutic services like vision, hearing and speech therapies, to name a few? The Yukon First Nation Education Directorate will only provide this information to Indigenous Services Canada as part of a Jordan’s Principle application. The Yukon First Nation Education Directorate will not provide this information to any other service providers or government entities. I understand that I can withdraw or revoke my consent by providing written notice to the Yukon First Nation Education Directorate.

Waiver, Release, and Indemnification Agreement

I understand that participation in this Event may expose the participant to risk, and to any dangers or hazards inherent in the planned activities.

Yukon First Nation Education Directorate cannot guarantee the participant's safety in respect of their participation in this Event. Nor can it control, or plan for each and every circumstance that may occur during their participation in this Event.

By participating in this Event, I agree that the participant is doing so at their own risk. In doing so, I understand that I am voluntarily assuming any risk of harm to the participant, and of any loss or damage to my property.

By signing this waiver, I waive any claim that I, or anybody claiming through me, might have against Yukon First Nation Education Directorate as a result of such participation. Should the participant suffer any harm to myself, or any damage to my property as a result of participating in the Event, even if such harm or damage is caused by the negligence of Yukon First Nation Education Directorate, I release Yukon First Nation Education Directorate from any resulting claim that I might have, and I do so on behalf of myself and anybody that might claim through me in any capacity.

To the extent that, notwithstanding the above waiver and release, Yukon First Nation Education Directorate might become liable to me, or to somebody else, on account of my participation in this Event, or on account of not being able to do so, I will fully indemnify Yukon First Nation Education Directorate for the full amount of any such liability incurred, including its costs of defending itself.

This Waiver, Release, and Indemnification Agreement is contractual in nature and does not consist merely of recitals.
