Membership Cancellation

Please use this form to cancel Taekwon-do, Muay Thai, and BJJ Memberships. 

Please send in a cancellation form for each members you wish to cancel. 

There are NO refunds on membership fees. 

Pre-paid memberships are non refundable. 

As per the 30 days cancellation policy, this form must be received 30 days before your next billing date to cancel for that date (ie: Cancel on July 2nd for August 2nd billing). If your cancellation is received with less than 30 days to your next billing date, the next billing will process and your membership will be valid until the end of the next 30 days. Please check your billing dates on your invoice so you know when you need to cancel.
