Workers Compensation Insurance

In the event of an injury that required medical care, the named volunteer is covered under Fair Oaks Recreation and Park District's Workers Compensation Insurance.

In the event the volunteer is injured, the volunteer will need to immediately report the injury to a Fair Oaks Recreation and Park District staff member. Volunteer and or guardian of volunteer acknowledges the volunteer is covered under Fair Oaks Recreation & Park District's Workers Compensation Insurance.

Photographic Release: I understand that photographs may be taken during this activity and hereby grant the Fair Oaks Recreation & Park District permission to use any such photo(s) for advertising or in promotional materials.

Waiver and Release of Liability

I understand and agree that submitting this application form does not automatically register me or my child as a volunteer, and that there may be certain qualifications I or child volunteer must meet, including the acceptance of established volunteer policies and procedures before I may begin volunteering.

By submitting this form, I attest that the information I have provided on the form is true and accurate.

In consideration for being permitted by the above District to participate in the above activity(ies), I hereby waive, release, and discharge any and all claims for damages for personal injury, death, or property damage which I or child volunteer may have, or which may hereafter accrue to me or child volunteer, as a result of participation in said activity(ies). This release is intended to discharge in advance the above District (its officers, employees, and agents) from any and all liability arising out of or connected in any way with my or child volunteers participation in said activity(ies), even though that liability may arise out of negligence or carelessness on the part of the persons or entities mentioned above. It is understood that this activity involves an element of risk and danger of accidents and knowing those risks I hereby assume those risks for myself and child volunteer. It is further agreed that this waiver, release and assumption of risk is to be binding by my heirs and assigns. I agree to indemnify and to hold the above persons or entities free and harmless from any loss, liability, damage, cost, or expense which they may incur as a result of my or my child's death or any injury or property damage that I or my child may sustain while participating in the said activity(ies).

I HAVE CAREFULLY READ THIS AGREEMENT, WAIVER AND RELEASE AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND THE ABOVE DISTRICT AND I AGREE TO IT OF MY OWN FREE WILL.
