As President of a minor hockey association of Hockey Manitoba, I understand that I am responsible for collecting registration information for all participants within our association. I also understand that each registration document signed by members must contain Hockey Manitoba’s privacy statement. I acknowledge that all supplemental documents such as copies of birth certificates, health #, etc. used to determine a new member’s age are required at the time of registration, however a copy of this information is not required on file.
I agree to securely store all membership information for a period of three years. In addition, I agree to destroy (shred) all registration information after the three year period. This information must only be used for hockey purposes as established under Hockey Manitoba’s privacy policy.
I understand that Hockey Manitoba reserves the right to request a copy of any membership registration documents at their discretion upon which our association will forward the requested information in a timely fashion.
I agree, to the best of my knowledge, that all registration information entered into the Hockey Canada Registry (HCR) system accurately reflects the information received by our minor hockey association.
