On Saturday, July 18, a new open water swimming event will take place on Île Notre-Dame: the Québec Open Water Grand Prix. For the first time in Canada, a 3 km knockout race will be featured, along with two short-distance events.

The athlete declares that they have had sufficient time to read and review this consent form and to ask any questions they deemed necessary in order to fully understand the scope of their rights and obligations.

Furthermore, the athlete declares that they are in good health and physically fit to participate in this open water swimming event.

The athlete acknowledges that they are aware of the risks and dangers associated with participation in an open water swimming event and freely and voluntarily agree to participate.

The athlete agrees to comply with all rules and regulations in effect for this event.

The athlete acknowledges that any false or inaccurate information provided, including with respect to their membership in the club identified above, may result in their disqualification from the event and may prevent them from participating in future open water swimming events sanctioned by the FNQ.

As a condition of participation in any open water swimming event, each member is expected to consent to being photographed and/or recorded on video. The member authorizes the FNQ and its recognized organizations to distribute, publish, and reproduce such photographs or videos (whether taken individually or in a group) solely for news, advertising, or promotional purposes. The member understands that, in this context, the FNQ and its recognized organizations shall not be held liable for the distribution, publication, or reproduction of duly authorized images. Furthermore, the member hereby waives any legal recourse and any claim arising from the distribution, publication, or reproduction of such photographs or images for the purposes stated above.
