VCA Waiver I, the participant, parent/guardian, hereby give my approval and permission for my participation/my child(ren)’s participation in all facets of the program(s). I hereby acknowledge and agree that my participation/my child(ren)’s participation in this/these program(s) is at my sole risk and liability. I hereby consent to any medical attention or other care or treatment deemed necessary by the Coach, Instructor, Manager or other responsible adult for myself/my child(ren). I hereby release and agree to hold harmless the Varsity Community Association, the Coach, Instructor, Manager or any other responsible adult from any and all actions, claims, liabilities and demands whatsoever arising out of or in any way related to or connected with this/these program(s) without limitation.
