Fair Oaks Recreation & Park District - Birthday Party Application

We look forward to hosting your child's birthday party with you!

Please submit the following application at least one month prior to your party. Once your form is submitted, recreation staff will contact you within 2-3 business days to confirm availability and discuss the booking process. Forms not submitted one month prior will automatically be denied unless program staff are able to accommodate.

How Old Are They Turning? *

Child must be turning 6-12 for all party rentals with the exception of the Fair Oaks Preschool. Children with the Preschool Party location must be turning 3-5.

What is the Birthday Party Theme? *

Total Number of Party Participants (Maximum 20) *

Party Participants include anyone participating in party crafts / games / activities. Children must be ages 6-12 for ALL rentals excluding the Fair Oaks Preschool. Fair Oaks Preschool party participants must be ages 3-5.

Total Number of Guests Invited *

This includes your party participants as well as additional family members, friends, etc. Please note: depending on maximum capacities of facilities, this number may be capped.

Date of Rental *

Rental must be requesting a booking at least 1 month in advance from submission date.

Time of Rental *

Must be Between 8AM-8PM | Minimum: 2 Hours | Maximum: 4 Hours

Party Time may need to be flexible depending on time of year, season, park closures, etc. Do not include set up and clean up in your request, as those are added on by default (60 min. setup, 30 min. for cleanup)

Birthday Party Package Rules & Regulations (please read before submitting)

Birthday Party Packages Start at $184 and include the following:
- Minimum of a 2-hour party, 1 hour designated to themed games and 1 hour to celebrate!
- 1-2 party hosts
- Rental Space
- Picnic Area and/or Tables & Chairs (depending on rental selection)
- Designated 60 minutes for set-up and 30 minutes for clean up before and after your rental respectively
- Additional Add-On's may increase fees
Upon submission of this form, you will hear back from District staff within 2-3 business days with a cost estimate based on your selections in the form, confirmation of availability, and submission of an invoice, which is required to be paid in full at least 15 calendar days prior to your party. In addition to the $184 base fee, a refundable $100 deposit will be required which will be refunded if the rental area is left clean, all rules were followed, etc. Refund is processed AFTER the party date. Failure to complete payment of your invoice at least 15 days prior to your event will result in immediate cancellation of your rental and contract.
Guests are limited to their designated activity theme and time only. Parties that stay past their designated time will be charged an additional $50 fee per 15 minute block.
Weather: We offer birthday parties year-round. Cancellations due to weather may result in a loss of part or all of fees collected unless the date can be rescheduled.
Decorations: Decorations are the responsibility of the renter to provide, set up, and clean up. Decorations may not include: loose glitter, confetti, rice, etc. Decorations must only be fastened with blue painter's tape or string, and may not be fastened with pins, nails, glue, or other harmful hardware.
Chaperones: It is required to have at least 2 adults (age 18+) in attendance at all times. FORPD staff are not responsible for direct supervision, oversight, or behavior management of children at any time during your rental.
Outside Food, Alcohol or Amenities: We do not provide any food, soft drink, dessert, or cutlery at parties. Food / Beverages & Supplies are the responsibility of the renter to provide and set up. Alcohol/glass is strictly prohibited, no exceptions.
Bounce Houses: With the exception of Gum Ranch Park Parties, you may rent and utilize a bounce house at your own expense during your rental per the District's bounce house policy. This includes choosing from the District's bounce house approved vendors and obtaining insurance. Additional details can be obtained after submitting this application.
Changes in the Agreement: All changes to the original agreement, including transferring dates must be approved before your “All Changes Final” date, which is 15 calendar days in advance. Additional fees may be required, and changes are subject to facility and staffing availability.
Cancellations/Refunds/Returns: Refund requests received 15 calendar days prior to the party will receive 100% refund. Requests received 8-14 days prior to the party will receive a 50% refund. Requests received 7 calendar days or less will NOT receive a refund. To reschedule to another date, a request must be made at least 15 calendar days prior to your original party date.
Other: Please do not tip our staff. We prefer you complete a program evaluation.
Fair Oaks Recreation & Park District is not be responsible for items forgotten, damaged, lost, or stolen.
