Child/Youth Into the Wild Release of Liability, Waiver of Claims, Assumption of Risk and Indemnity

    
Town of Banff and its respective elected officials, officers, directors, employees, volunteers, partners, and contractors (collectively referred to as the “Releasees”).

ASSUMPTION OF RISKS

I, the undersigned, in consideration of my child/ward’s participation in the “Into the Wild Program” at any point on and after the date of my signature indicated below, offered by the Town of Banff out of the Rundle Cabin in Sundance Park and all other locations;  I hereby accept that my child/wards participation in the Into the Wild Program come with inherent risks, which include but are not limited to: slips, trips and falls, cuts, burns, abrasions, failing to engage in activities safely or within one’s own ability, infectious diseases transmitted through direct or indirect contact, unsafe actions of other program users, transportation to and from the program, negligence of other persons and NEGLIGENCE ON THE PART OF THE RELEASEES, death and property damage, or other loss. I understand that it is my responsibility to learn about and understand the risks, dangers, and hazards of my child’s participation in the activities related to the Into the Wild Program and accept the risk associated with my child/ward’s participation.

PERMISSION TO PARTICIPATE

In consideration of the releasees allowing my child/ward to participate in the “Into the Wild” program, which includes all activities, services, use of facilities, programs, orientations, instructional sessions such as outdoor educational and wilderness survival skills which include the use of sharp knives, matches, lighters, fire and flammable materials, and other activities offered by the Releasees, I hereby agree as follows:

In entering this Consent and Acknowledgement of Risk Agreement, I am not relying on oral or written representations or statements made by the Town of Banff, its agents or employees with respect to the safety of the Into the Wild program other than what is outlined in the Consent and Acknowledgement of Risk Agreement.

This personal information is being collected under the authority of the Freedom of Information and Protection of Privacy (FOIP) Act, Section 33(c). This information will be used in the administration of this program.
