In consideration of your accepting my registration and participation in the sailing and boating program, I hereby, on behalf of myself, my child(ren), my heirs, executors and administrators, waive and release any and all rights and claims for damages I may have against Longshore Sailing School and its' employees, representatives, successors and assigns, for any and all injuries suffered by myself as a result of participation in the program. I understand there is inherent risk associated with the activity and willingly assume these risks. I further authorize emergency medical treatment and transportation in my absence.
Photo release: Longshore Sailing School may videotape or photograph participants engaged in its programs and activities. These images may be used for promotional purposes.
