Grades 1 to 6 (based on September 2026 grade level)

Designed for Grades 1–6, this engaging program combines educational enrichment with plenty of fun. Students will explore a variety of subjects, including Science, STEM, Arts, Debate, and Public Speaking, while participating in exciting indoor and outdoor activities led by experienced instructors.

The goal of the camp is to create a positive and memorable summer experience where students can have fun, build friendships, and continue learning in a supportive environment. 🎉

Liability Waiver & Consent Form*

I, the undersigned, hereby grant permission for my child, who is enrolled in the DebateOn Leadership Academy Summer Day Camp 2026, to participate in all camp-related activities, including but not limited to instructional sessions, workshops, recreational activities, and any off-site or outdoor activities organized by the academy.

I acknowledge and understand that participation in camp activities may involve certain inherent risks, including but not limited to physical activity, group interaction, and travel between locations (if applicable). I voluntarily assume all such risks on behalf of my child.

I agree, on behalf of myself and my child, that DebateOn Leadership Academy, its directors, including David Jung and Ally Jung, as well as its staff, coaches, volunteers, officers, and affiliated representatives, shall not be held liable for any injury, illness, loss, or damage to my child or their personal belongings arising from or in connection with participation in the Summer Day Camp.

I understand that while all reasonable precautions and supervision will be provided to ensure the safety and well-being of participants, the academy and its representatives cannot guarantee absolute safety and shall not be held responsible for unforeseen incidents or accidents.

Medical Authorization
In the event of a medical emergency, I hereby authorize DLA staff, coaches, or designated representatives to seek and obtain appropriate medical attention for my child, including transportation to a medical facility if necessary. I understand that reasonable efforts will be made to contact me or the emergency contact listed prior to taking such action.

My signature below constitutes consent for any necessary medical treatment in the event that I cannot be reached in a timely manner.

Code of Conduct & Participation
I acknowledge that students are expected to follow all camp rules, guidelines, and instructions provided by DLA staff. Failure to comply with these expectations may result in dismissal from the program without refund.

Photography & Media Consent 
I grant permission for photographs and/or videos of my child taken during the Summer Day Camp to be used for promotional and educational purposes by DebateOn Leadership Academy (e.g., website, social media, marketing materials).

Acknowledgment
By signing below, I confirm that I have read, understood, and agreed to the terms outlined in this Liability Waiver & Consent Form. I acknowledge that this agreement is legally binding.

Payment Confirmation and Refund Policy

  • Please note that enrollment is only secured upon receipt of full payment, and all fees are non-refundable.
  • Payment must be completed by the specified deadline in order to qualify for any applicable promotions or rates. Should you have any concerns regarding payment, please contact the administrative team. We are happy to discuss flexible arrangements to best accommodate your needs.

Withdrawal Policy

  • Prior to the first day of the program: A $100 non-refundable deposit will be retained; the remaining balance will be refunded or issued as a credit.
  • After the program has commenced: All tuition fees are strictly non-refundable.

Payment Terms

  • Late Fees: A $100 late fee will be applied if payment is not received after three formal notices.
  • Accepted Payment Methods: Payments may be made via E-transfer, credit card, cash, or cheque. Timely payment is required to ensure uninterrupted participation. (Please note that a 3% processing fee applies to credit card payments.)

Refund Policy

  • Cancellations by DLA:
    In the event that DLA cancels a class or program for any reason, participants will receive either a full refund or a credit toward a future program, depending on the nature of the cancellation.
  • Personal Absences:
    If a student is absent from a class or workshop due to personal reasons and fails to provide at least 24 hours’ prior notice, no refunds, credits, or make-up sessions will be issued.

Should you experience any exceptional circumstances or require further clarification regarding payments or refunds, please contact the DLA administration. We are committed to supporting our families and ensuring a positive experience for all students.
