We’re so excited to have your child join us for an evaluation class!
This experience helps us get to know your child, introduce them to aerial yoga, and determine the best class fit — all while having fun in the clouds ☁️

Evaluation Fee Information
The $25 evaluation class fee will be applied toward the $50 registration/new membership fee if your family decides to join.

Things to Know Before Class

What to Wear: Comfy clothes your child can move freely in
Grippy socks are required (available for purchase at the studio)
No jewelry (studded earrings okay)
Hair pulled back and secured
Please bring a bottled water

Intake/Waiver: Assumption Of Risk, Consent & Liability

IN CONSIDERATION of allowing my minor child to participate in Kids Aerial Yoga, provided by Zen Out Events, LLC DBA Grounded in the Clouds

1. I am a parent/guardian having full legal responsibility for decisions regarding my minor child.

2. I believe that my minor child is physically, emotionally and mentally able to participate in this program.

3. I understand, and will instruct my minor child, that all applicable rules for participation must be followed at all times.

4. I will immediately remove my minor child from participation, and notify Grounded in the Clouds, if at any time I become aware of or have reason to believe that my minor child has experienced any deterioration in his/her physical, emotional or mental fitness for continued participation in the program.

Contraindications for Aerial Classes: Recent surgery, cardiac irregularities, some spinal issues, glaucoma, vertigo, very high or very low blood pressure.

Agreement: I, the undersigned, acknowledge that my child is participating voluntarily in an event/class offered by Zen Out Events, LLC (DBA Grounded in the Clouds). I recognize that the event/class activities may cause physical and/or mental strain or distress, and may result in damages including but not limited to physical injury, property loss, or death. I am fully aware of the hazards involved and consent to my child’s participation with the known assumption of such risk. I acknowledge and agree to waive any right or claim for damage arising from any event or class including, but not limited to, any bodily or physical injuries, mental anguish, or death. I further acknowledge and agree not to sue or make any other claims against Zen Out Events, LLC (DBA Grounded in the Clouds), its members, owners, and employees, for any personal injury, property damage, loss, or wrongful death. I understand the need to promptly disclose any pre-existing medical or other condition that may affect my child’s risk of injury, loss, or damage.
