1. Only one form needs to be submitted per person.
2. Please complete all required options & click next until you complete the entire form.
3. You MUST click SUBMIT on the last page in order for the form to be received.
4. Please contact management if you are unsure of how many authorized hours you currently have!!!
5. Don't forget to download a PDF after completing & to set reminders for yourself for the activities that you sign up for. (You can add it to your calendar, set a reminder on your phone, or write it down somewhere that you won't forget it!)
6. Please be considerate and avoid cancelling once you commit to an activity.
