Waiver and Release:

I, [Parent/Guardian’s Full Name], as the parent or legal guardian of the above-named child, acknowledge and agree to the following:

Assumption of Risk: I understand and acknowledge that participating in kids yoga involves inherent risks, including but not limited to physical injury. I voluntarily assume all risks associated with my child’s participation in this activity.

Release of Liability: I hereby release, waive, discharge, and covenant not to sue Active Sprouts and its officers, employees, volunteers, and agents (collectively “Releasees”) from any and all claims, demands, losses, or damages arising out of or related to my child’s participation in the activity, including claims for personal injury, property damage, or wrongful death, whether caused by the negligence of the Releasees or otherwise.

Medical Treatment: In the event of an emergency, I authorize Active Sprouts to seek medical treatment for my child if necessary. I understand that I will be responsible for any medical costs incurred.

Photographs and Media: I consent to the use of photographs or video footage of my child taken during the activity for promotional purposes by Active Sprouts. I understand that these images may be used in various media, including social media, websites, and print publications.

Code of Conduct: I agree to ensure that my child adheres to all rules and instructions provided by Active Sprouts. I understand that failure to follow the rules may result in my child being removed from the activity without refund.

Acknowledgment of Understanding: I have read this waiver and release form and understand its contents. I am aware that by signing this document, I am waiving my right to bring any legal action against Active Sprouts.
