WAFL Equipment Report Form

This report form is for groups who have borrowed skis, snowshoes or kicksleds from the WAFL Equipment Library. Groups submit a short report by March 31st. Completing the report is required to maintain eligibility to borrow from the WAFL Library.

Please write accurately and clearly as the report will be shared with board members, funders, etc.
