I, the undersigned applicant for employment with the Park District of Forest Park ("District") hereby authorize the District, through its authorized personnel or persons authorized by them, including, without limitation, companies which perform background investigation services and provide reports based thereon under contract with the District and law enforcement agencies which perform such investigative services and provide reports based thereon (the District, its authorized personnel, or any other persons authorized by them being hereinafter collectively referred to as the "District Personnel") to conduct any and all investigations (including, without limitation, a criminal conviction investigation) and make any and all inquiries, as they shall deem necessary, desirable or appropriate to determine my apparent suitability and fitness for the position for which I have applied, and/or to verify any information which I have provided or may hereafter provide in connection with my application for my employment. I hereby further request and authorize each of my prior employers to answer any and all oral or written inquiries from any District Personnel concerning my prior employment. In order to induce my prior employers to release such information to District Personnel, and in consideration for the District's accepting my application and considering me for the position for which I have applied, I hereby fully and without condition release and hold harmless each such prior employer and District Personnel from and against any claims, suits or proceedings which I or anyone claiming through me might have by virtue of the disclosure of such information by any of my prior employers or by virtue of the obtainment of such information by District Personnel. I have carefully read the provisions of this authorization form, fully understand its contents and significance, and have voluntarily signed it.
