Waiver, Release of Liability and Consent Form

I want to participate in events organized or sponsored by Sports en français, including the event indicated above. In exchange for the permission to participate and use any equipment provided during these events, I agree to the following:

Risks

My participation could lead to injury/death or damage/destruction of property. I am fully and solely responsible for my safety during my participation in the events, and I accept and assume all risks, known and unknown, associated with my participation in the events.

I understand that the instructions given through classes are issued for guidance only. I understand that while all necessary precautions are taken by the instructor, he/she cannot be held responsible for my incorrect practice at any time. To ensure that no injury occurs, I agree to adapt my practice according to my limits, and the decision to perform yoga postures remains mine. I declare that I take full responsibility for myself during classes. I will inform my instructor before each class of any recent injury, illness, surgery or pregnancy.

Physical Condition

I am physically fit to participate in the events, and I have no medical condition that would impair my ability to participate in the events or endanger any other participant in the events.

Waiver, Release and Agreement Not to Sue

I release and discharge Sports en français and its administrators, officers, employees, volunteers, sponsors and representatives from any damage, claim, injury, liability and cost (the "Claims") related to my participation in the events and use of equipment, including all Claims arising from negligence by Sports en français or any other individual mentioned above, and I agree not to sue Sports en français or any individual mentioned above for any of these Claims.

Release of Liability

I will indemnify and hold harmless Sports en français and the individuals mentioned below from any liability for all Claims of whatever nature arising from or related to my participation in the events or use of equipment, including any damage or injury that I may cause to a third party or their property, as well as any damage or injury that may be caused to me or my property by a third party.

Photos and Videos

I grant consent to Sports en français and its representatives and contractors to take photos and videos of me during the events, and modify, publish or otherwise use said photos and videos in any manner whatsoever including compilations, publications, catalogs, posters, banners on websites; in advertisements for products/services; and for other commercial purposes. I waive any rights I may have over such photos/videos including rights of inspection or approval regarding their use. I also acknowledge that I will receive no compensation regardless of how my photos/videos/name/image are used.

Miscellaneous

I have reached the age of majority in the province where these events are offered. I understand that this acceptance applies to all activities/events/programs supported/sponsored/and/or conducted by Sports en français. This agreement binds me as well as my heirs/administrators/reps. I've had an opportunity to seek legal advice before signing this document; however I've voluntarily chosen not to do so.
