I, the participant, or parent / legal guardian of the designated above-named participant, acknowledge that participation in dance classes, rehearsals, performances, and related activities at Aria Dance Academy LLC involves inherent risks, including but not limited to physical and emotional injury, falls, sprains, strains, and other potential hazards. I recognize that severe injuries can occur in any activity involving height or motion, including dancing, stretching, conditioning, tumbling, and physical activity in general.

I understand that it is the expressed intent of all Aria Dance Academy LLC staff and personnel to provide for the safety and protection of myself or the above-named participant(s) and in consideration for using these facilities, or allowing the above-named participant(s) to use these facilities, I hereby voluntarily COVENANT NOT TO SUE and FOREVER RELEASE Aria Dance Academy LLC, this facility, affiliated and partner companies and organizations, property owners and lessors, staff, contractors,
subcontractors, teachers, coaches, owners, directors and other members involved in this facility's program(s), from all liability and for any and all damages and injuries suffered by myself or the above-named participant(s) during instruction, supervision, and/or control during any and all classes or extra activities, whether caused by negligence or otherwise.

MEDICAL AUTHORIZATION: In the event of an injury or medical emergency, I authorize Aria Dance Academy LLC and its staff to seek medical treatment for myself or the above-named participant(s) at my expense. I understand that I am responsible for any medical costs incurred as a result of participation.

There will be 36 weeks of instruction and classes throughout the year, divided into 3 equal payment terms.
Late registrations will be prorated to reflect missed classes
To secure a spot in a class, clients must include a credit card number with all online registrations. This card information is safely stored by Stripe and installments are charged to the number on file.
Clients paying by Visa or MasterCard will be automatically billed on Aug. 18, 2025 (for regular prices) or today’s date (if registering after Aug. 18). Two additional installments are automatically charged on Nov. 17, 2025, and Feb 23, 2026.
If a student wishes to withdraw from a class, Aria Dance Academy must receive written notice 14 days before the next term begins: by Aug. 18, 2025 for Term 1 – by Nov. 17, 2025 for Term 2 – by Feb. 23, 2026 for Term 3. If written notice is received, checks will be returned, and/or cards will not be billed. Exceptions may only be made to Aria Dance Academy’s refund policy if a doctor’s note has been received.
If a student is switching to another dance class, program transfers can be accommodated as long as the move is made before Jan. 1, 2026, and providing there is space available in the new class.
If a student has pre-registered but chooses to withdraw from classes after Jun. 1, 2025 and before Aug. 18, 2025, full tuition will be refunded, less a $25.00 processing fee for EACH class registered.
A $100 / class recital costume fee is due upon registration and collected on Dec. 1, 2025. If a dancer will not be participating in the recital, Aria Dance Academy must be notified by Dec. 10, 2025, for a full refund to be issued.
