Trial/Enrollment Agreement

1. The undersigned parent or guardian (Parent) of the above child agrees to pay the SNACK (Program) the non–refundable semester fees (Not applicable for trial class). All previous semester fees must be paid in full prior to enrolling for the upcoming semester. A $50.00 registration fee applies for each semester enrollment. One invoice will be issued for the entire semester. An email containing the payment instructions and link to make a direct payment will be sent to you from our accounting@snacknyc.com email address. For your convenience, we have set up a payment schedule: 50% due upon enrollment, 50% due mid semester (approximately 6 weeks later).
2. The Parent understands that the time and date for each of the sessions has been predetermined by the program and provided in advance to the parent. Schedules and fees are billed on a semester basis (approximately 12 weeks), we ask that you enroll for the entire semester. If you know of a date that your family member will not be attending, such as a vacation, respite, holidays, or family functions, please let us know prior to enrollment. You will be responsible for the fees for any missed classes.
3. The Parent understands that there is no credit or make–up given for missed or unused sessions and that all fees paid to the programs are non-refundable. If a child withdraws from the program at any time up to the two weeks prior to the date of the first session, a full refund will be provided. If a child withdraws once the program has begun, there will be no refunds for any unused portion of the fees paid.
4. The parent understands and agrees that the Directors of the program have the right to terminate the enrollment of any child from the program at any time if the Director, in the sole discretion, determines that the continued attendance of the child in the Program is not in the best interest of the child or the Program. The undersigned acknowledges and agrees that the child’s enrollment may be terminated for a violation of the rules or regulations of the program, at the Program’s sole discretion, without any refund or cancellation of fees paid, which the undersigned nevertheless agrees to pay in accordance with the terms of this contract.
5. In the event that the Parent cannot be reached and a situation arises which the Program Directors define as an emergency, the Parent hereby grants permission to the Program Directors to secure and provide for the necessary medical treatment, including hospitalization.
6. The Parent understands that the Program does not have the service of a nurse or other medical personnel on premises to administer medication or provide medical treatment. The undersigned acknowledges and agrees that, to the extent that the child is required to be administered medication and /or require first aid during the time that the child is in the Program, that non-medical staff would administer such medication and /or provide first aid
7. I/we the Parent of the above child understand that part of the experiences that my/our child will be having may be new to my child and they come with certain risks and uncertainties beyond what my child may be used to dealing with at home or in his/ her school environment. I/we realize that no environment is risk-free, and we are prepared to assume on behalf of our child, the risk involved in his/her participation in the SNACK Program. The Parent furthermore releases and holds harmless the program, its agents and employees from all claims, damages, or other liability for injury to the child where such claims, damages, or other liability is not responsible for any medical expenses for the child.
8. At the sole discretion of the Directors, a neuro-typical sibling of an enrolled child is permitted to attend any SNACKtivities session that the enrolled child attends at no additional cost to the parent. Arrangements must be made prior to the intended session. The terms of paragraphs 4,5,6,7, and 8 of this enrollment agreement apply to the non-paying sibling.
9. The Parent will provide a copy of the above child’s current school behavior plan. SNACK also has permission to contact the child’s school with any questions and/or concerns regarding the provided behavior plan. The provided behavior plan will have no effect on the enrollment of the above child.
