TWISTERS MEDIA RELEASE & PRIVACY FORM

I have read the information provided and give Twisters Gymnastics permission to use my photos/videotape or digitally stored images for the purposes stated below while my son/daughter is a registered participant with Twisters Gymnastics Recreational/Camp Program.

Twisters’ staff or designated volunteer(s) may photograph, videotape and/or digitally store images of individual athletes or groups of athletes to promote special events taking place at or hosted by Twisters Gymnastics.
Static or electronic display may be used to promote Twisters Gymnastics programs, competitions or fundraising efforts at various sites, including the entrance to Twisters Gymnastics Club. The athlete’s names, age, picture and comments may be published in the local newspapers.
Twisters Gymnastics must comply with Freedom of Information and Protection of Privacy legislation, which places great emphasis on protecting the privacy of our individuals. In keeping with this legislation Twisters must have your consent to use information for any of the purposes listed above and for any other release of personal information. Inquiries regarding personal information, from any outside source, will be referred to the Recreational Director and or Club Director.
