Welcome to the secure, online complaint form for Flicka Gymnastics Club. This form allows you to make a complaint or provide feedback to Flicka regarding our classes and programs.
Flicka recognizes that you may be making your complaint or report about unpleasant, upsetting, and/or disturbing experiences at our gym. While completing this form, you will be asked to recount details of that experience, which can cause emotional distress. We recommend that you complete this form somewhere private where you feel safe. The form is set up to allow you to save it and come back to it later, should you wish to complete it in smaller time chunks.
WHO CAN I SUBMIT A COMPLAINT/REPORT ABOUT?
After you submit this form, we will first consider whether the complaint is made against a person who is (or was) under the jurisdiction of Flicka Gymnastics Club as an employee. Also, we will evaluate if the details provided pertain to a class or program at Flicka and are substantial enough to help us make informed decision about the complaint.
Flicka will do its best to address all complaints promptly and fairly.
Note: A complainant is the person submitting the complaint.
