CONSENT AND ACKNOWLEDGMENT OF RISK - Minor Participant (Hereinafter “Consent and Acknowledgement of Risk Agreement”)

BY SIGNING THIS DOCUMENT, YOU ACKNOWLEDGE THE RISKS OF THE MOUNTAIN ADVENTURERS PROGRAM

Town of Banff and its respective elected officials, officers, directors, employees, volunteers, partners, and contractors (collectively referred to as the “Releasees”).

ASSUMPTION OF RISKS

I, the undersigned, in consideration of my child/ward’s participation in the “SSBV Try It Sports Program” which includes all of the programs - current and future listed HERE at any point on and after the date of my signature indicated below; I hereby accept the inherent risks associated with such participation, which include but are not limited to: slips, trips, and falls, cuts, abrasions, infectious disease contracted through viruses, bacteria, parasites and fungi which may be transmitted through direct or indirect contact, transportation to and from the program; negligence of other persons and NEGLIGENCE ON THE PART OF THE RELEASEES, death and property damage, or other loss. I understand that it is my responsibility to learn about and understand the risks, dangers, and hazards of my child’s participation in the activities related to the SSBV Try It Sports Programs and accept the risk associated with my child/ward’s participation.

PERMISSION TO PARTICIPATE

In consideration of the releasees allowing my child/ward to participate in “ SSBV Try It Programs”, which include all activities, services, use of facilities, programs, orientations and instructional sessions, classes and clinics, events and other activities offered by the Releasees, I hereby agree as follows:

In entering this Consent and Acknowledgement of Risk Agreement, I am not relying on oral or written representations or statements made by the Town of Banff, its agents or employees with respect to the safety of the SSBV Try It Sports Program other than what is outlined in the Consent and Acknowledgement of Risk Agreement.

I AM AWARE OF THE RISKS, DANGERS, AND HAZARDS ASSOCIATED WITH MY CHILDS/WARDS PARTICIPATION IN THE TRY IT PROGRAM, AND I FREELY ACCEPT AND FULLY ASSUME ALL SUCH RISKS, DANGERS, AND HAZARDS AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, PROPERTY DAMAGE, OR LOSS RESULTING THEREFROM.
