Please read the following clauses carefully before signing the form.
Payment
You must pay online, by bank transfer or credit card, in a single payment before the start of classes.
Cancellation Before Classes Start
If you cancel your registration before classes start, LADC will issue a full refund.
Cancellation After Classes Start
If classes have started, you will need to pay these 2 amounts :
- the cost of classes already attended;
- a penalty provided by law. This penalty corresponds to the lesser of the following amounts : $50 or 10 % of the price of the classes that were not attended.
Class Cancellation
If a class needs to be canceled, LADC will make efforts to arrange another class in its place, except in cases of cancellation due to bad weather or power outages.
If a government authority orders the closure of dance studios for a certain period, classes will be immediately transferred online during the required period. Replacement online classes will be considered an equivalent service and no refunds or discounts will be granted.
Liability and Safety
LADC disclaims any liability for injury, loss or theft incurred during classes or on premises.
Students participate in classes at their own risk (or that of their legal guardian for minors).
Parents must ensure that their child is in good physical condition to participate in activities.
Medical Consent
In case of emergency, I authorize school officials to provide or arrange for necessary first aid for my child or myself.
I assume responsibility for any costs related to urgent medical intervention if necessary.
